 o   Every student must have their CNS consent form with them and it must be signed by a parent. Form available at the school.

o   Every student must have a printed copy or photo image of the parent’s ID who signed the form and the signature must be visible/readable.

o   There may be an opportunity for parents to get a shot with their student. Those who are interested should ask the school to put them on a stand-by list. If we know about them, we can plan for them as well.
